Job Description: Provides intensive case management to individuals with chronic and persistent mental illness. These services are provided within an individual and/or group context.

Education

Bachelor Degree or Diploma in Nursing or Psychiatric Nursing

Bachelor of Psychiatric Nursing (BPN)

Licenses

Valid Class 5 driver's license

Other Education and Training

Psychiatric Nursing diploma is acceptable

Nursing diploma is acceptable

Bachelor of Nursing is acceptable

Knowledge and Abilities

Knowledge of community resources including treatment programs & support agencies

Knowledge in principles/programs/practices in community mental health nursing

Knowledge of factors affecting high-risk populations and their family dynamics

Demonstrated cultural sensitivity and openness to working w/ diverse populations
